Communication Request Form

This form is for events and ministry initiatives that need communication in the church and community. If you are completing this form for an event make sure that you have completed the Espace Facility Reservation and created any Registrations needed prior to completing this form. Allow 5 weeks lead time before your event (or the registration open date) for communications. These projects involve graphic design, copy writing, editing/proofing, web design etc. The lead time is necessary to allow adequate time for quality work.


Please have the form filled out in it’s entirety with all the necessary information for your event/ministry initiative. An incomplete form will delay the start of your communications.

Think of this as practice for now - this piece will change once Identity & Direction conversation is complete.

List person/people and brief description of the story. We will work together to capture the story. If this is a new event please note in this field.

List full dates, locations and timing for your event/initiative. Make sure to note if this event is reoccurring. If recurring, list any dates that you will not meet.

This doesn't guarantee availability of photo/video.

Graphic Design

Please list all supporting graphics needed for your request. This list should include any other graphics needed beyond the primary graphic set for your event (i.e. if there is a program, mailer etc.)
