Communication Request Form

This form is for events and ministry initiatives that need communication in the church and community. If you are completing this form for an event make sure that you have completed the Espace Facility Reservation and created any Registrations needed prior to completing this form. Allow 5 weeks lead time before your event (or the registration open date) for communications. These projects involve graphic design, copy writing, editing/proofing, web design etc. The lead time is necessary to allow adequate time for quality work.


Please have the form filled out in it’s entirety with all the necessary information for your event/ministry initiative. An incomplete form will delay the start of your communications.
